• Pays money directly to you for injuries that happen on or off the job
• Wellness annual benefit -$60 for physical, Pap smear, Mammogram, eye exam, etc.
• $120 paid for first doctor visit plus $35 for each follow-up/physical therapy
• $1000 hospital admission benefit plus $250 each day charged for a room
• $2000 ICU benefit plus additional $400 per day up to the first 15 days; $250 daily thereafter
• Additional lump sum paid based upon specific injury
• Ambulance, crutches, wheelchairs, travel expenses, hotel bills, etc.
• Accident death benefit of $40,000 - $150,000
• Must be age 18-64 to enroll, but once enrolled plan guaranteed for life
