Educational and Employment Verification
(Formerly known as the CDA Waiver Program)


Qualifications

If you have an A.A., A.S. or higher degree in any field, you qualify to complete this program.

Who needs it?

Licensed childcare centers require one CDA qualified teacher per 20 children; this program will exempt you from having to complete a CDA Program. Centers are also required to have one person with a Director's Credential, the credential requires a CDA.

Course Requirements

If your degree is outside of the Early Childhood field, you will need to complete the following courses to complete the program:

  • EEC1000 - Child Growth and Development
  • EEC2200 - Educational Practices in ECE

If you have already completed a degree in Early Childhood Education, or after you have completed the above courses, please download and print the application form:

  • Go http://myflorida.com/childcare/training
  • Click on "Child Care Training Forms" (your computer will be required to have Adobe Reader to view and print this form).
  • Fill out the "Education and Employment History Verification Form" (Form 5211)
  • Submit the completed form to Tallahassee with all appropriate documentation.
  • You will receive a 5206 form after all qualifications have been met.

Please note that the documentation of 480 hours of childcare experience must be in a childcare setting. Elementary school experience does not count unless it is with preschool aged children.
