Time Table Deadlines for Submission of Health Information Packet

Health Information Packet

With the exception of the required pre-participation sports physical and proof of primary health insurance coverage, the deadlines listed in below apply to the Health Information Packet.

After Thursday, July 17, 2008	Information received and accepted or postmarked no later than two weeks after signature date of scholarship/LOI

All forms must be properly filled out, signed and dated. No athlete will be cleared for participation until all required information is received and accepted.

Pre-Participation Sports Physical

All athletes are required to receive an annual pre-participation sports physical. Please refer to chart below for submission deadlines of your pre-participation sports physical.

This form must have the date of physical, practice name, physical address and contact information and signature of the treating physician. No athlete will be cleared for participation without a properly filled-out and signed pre-participation sport physical.

Proof of Primary Health Insurance Coverage

All athletes must show proof of primary health insurance coverage (outlined in Form E2, Sections 3, 4 and 5). Proof of primary medical insurance is requested by the submission dates outlined in Table One above. Proof may be submitted upon reporting for the academic year; however, all insurances are subject to verification. No athlete will be cleared for participation until insurance has been verified.
