Downloads for the SCC Athletic Training and Sports MedicineCurrent and future student athletes of Seminole Community College are invited to utilize this resource page. Here, the student athlete will find links to the necessary documentation required by Seminole Community College Athletics and its regulatory committees. This documentation is necessary for competitive eligibility. These files have been created in Adobe Acrobat format and require the Adobe Reader Plug-in. This plug-in is preinstalled on most new browsers. If you need the plug-in you can download it free here. If you need instructions on downloading and installing the plug-in you can get them here. Once the file is open in your browser, use the print button on the plug-in screen for best results. Note: Some of these files are large and may take a few moments to download. Health Information PacketAll athletes, new and returning, must complete and submit a Health Information Packet prior to being cleared for participation in their recruited sport. Below you will find a link to the complete packet as well as links to the individual form sets. These files have been updated for the 2008 - 2009 school year. Addendums will be posted as a separate link following the list of original documents below. Those athlete who sign with SCC Athletics prior to Thursday, July 17th, 2008 are required to submit their Health Information Packet by Thursday, July 31, 2008. Those athletes who sign after July 17th, 2008 are required to submit their Health Information Packet no later than two weeks after signature.
